Registration Fee

The registration fee for participants is US $ 550.  This includes Symposium materials, charges for the conference facilities, accommodation (six night stay, double occupancy), continental breakfast at the Student Union, all lunches, dinners and the coffee breaks.  It also includes a Pittsburgh Pirates baseball game OR a trip to “Fallingwater”, a Frank Lloyd Wright house located approximately 60 miles from the city.  The website for Fallingwater is http://www.wpconline.org/fallingwaterhome.htm.  There is an additional charge of US $ 305 for those desiring single occupancy.   For accompanying persons, the fee is US $ 500 that includes a shared room, breakfast, lunches, dinners and social events.  The registration fee for those who do not need accommodation is US $ 250 which includes everything mentioned above, except the hotel.  It will greatly facilitate organization of the meeting if you register by April 3, 2002.  The deadline for submission of abstracts for poster presentation has been extended to April 3, 2002.
 

We strongly urge everyone to register through the University of Pittsburgh Medical Center’s SECURE Web Site located at (https://www.upmc.edu/ccehs/cme/formal_courses.asp)  and pay the registration fee using a credit card.  If this is not possible, you can still fill out the registration forms on the web site, print the off-line manual registration form and send your registration through Fax or mail. As you fill out these forms on the web site, you will be provided these instructions. In short, fill out the registration forms from the above web site and mail along with a check/draft payable to UPMC Center for Continuing Education to:

Accommodation

Accommodation for the participants has been arranged at the Holiday Inn Select, Pittsburgh University Center, 100 Lytton Avenue, Pittsburgh, PA 15213, USA.  The hotel is located in the Oakland section of Pittsburgh on the campus of the University of Pittsburgh, and is a 5 minute walk from the Student Union Building where the symposium will be held.  All rooms are double occupancy and contain two double beds.  Those desiring a specific roommate should send an e-mail to plas2002@pitt.edu specifying the name of their roommate.  Both smoking and nonsmoking rooms are available.  Please specify your preference on the registration form.  The hotel has an indoor swimming pool, an exercise room and a sauna.  These amenities are available to you at no charge.  The phone number for the hotel is 1-412-682-6200 and the Fax is 1-412-682-5745.  The hotel has a shuttle bus service that can be used, free of charge, to travel to the downtown and other nearby areas.  For those with a car, free parking is available at the Holiday Inn garage.
